is in your knowledge that the affiliation of your institution with the Board of
Intermediate
& Secondary Education, Faisalabad has been expired, so you are required to submit your
Affiliation bise fsd case for affiliation till 31-03-2020 along with the following documents otherwise your
institution’s online code will be blocked at the time of 9th registration for the session
2020-2022
1.
Application for extension in Affiliation bise fsd.
2.
Forwarding letter of Chief Executive Officer (Edu.)
3.
Inspection fee @ Rs. 12,000/- per year.
4.
Registration
certificate issued by the concerned Education Authority.
5.
Compliance
report of previously imposed conditions (if any).
6.
Fresh Staff Statement.
